چکیده

physiological changes in epithelial form and function(1). It was a pre-malignant disease of oesophageal adenocarcinoma (OA) in which normal oesophageal epithelia were replaced by columnar epithelia at gastro-oesophgeal junction(2-8). The increasing incidence of BO and OA over the last two decades shows that it was becoming a major public health concern in developed countries such as the US, UK and north of Iran. BO was identified sporadically in 10% of patients suffering from chronic gastrooesophageal reflux disease (GORD)(9). Many studies have been carried out with respect to the factors involved in the cellular and molecular processes associated with the replacement of squamous epithelial cells with columnar cells. Caucasian or Hispanic race, lifestyle and male gender are recognised risk factors(5,6,10,11). Review of Barrett’s Oesophagus to Oesophageal Adenocarcinoma: New Prospect

Oesophageal cancer and gastro-oesophageal reflux: what is the relationship?

T he direct relation between reflux and adenocarcinoma of the oesophagus has recently been addressed in several well designed population based epidemiological studies, using different study designs. The studies are all in agreement that reflux is associated with an increased risk of oesophageal adenocarcinoma in a seemingly dose dependent manner. The specific role of Barrett’s oesophagus for th...

Retinoids, bile acids and PPARs in Barrett's oesophagus.

Intestinal metaplasia of the oesophagus (Barrett’s oesophagus) is the most important risk factor for oesophageal adenocarcinoma. Although the role of mixed bile and acid gastro-oesophageal reflux is well established, the contribution of nuclear receptor signalling in the pathogenesis of Barrett’s oesophagus remains largely unknown. The recent article by Chang et al. (Gut 2007;56:906–17) showing...

Barrett’s oesophagus: Current controversies

Oesophageal adenocarcinoma is rapidly increasing in Western countries. This tumour frequently presents late in its course with metastatic disease and has a very poor prognosis. Barrett's oesophagus is an acquired condition whereby the native squamous mucosa of the lower oesophagus is replaced by columnar epithelium following prolonged gastro-oesophageal reflux and is the recognised precursor le...

Endoscopic surveillance of patients with Barrett esophagus.

I propose that a patient with a clinical diagnosis of Barrett’s oesophagus (BO) must have regularly endoscopic surveillance and protocol biopsy. BO is defined by endoscopically visible oesophageal columnar epithelium with intestinal metaplasia. The purpose of screening is detection of dysplastic change and early cancer, to allow early intervention and prevention of the suffering of symptomatic ...

The columnar lined oesophagus: a riddle wrapped in a mystery inside an enigma.

Just a few years ago, the definition and pathogenesis of Barrett’s oesophagus seemed straightforward. Barrett’s oesophagus was the condition in which metaplastic columnar epithelium replaced oesophageal squamous mucosa that had been damaged by exposure to refluxed gastric juice. The condition was sought primarily in patients with gastro-oesophageal reflux disease (GORD), and was identified when...
